Physician offices designated by the patient at registration as their PCP will receive records of the patient’s visit if Your Doc’s In has the physician offices’ fax or email address once the record is complete.
For non-PCP physician offices and attorneys, please request records by emailing medicalrecords@yourdocsin.com with the following patient information:
- Patient first and last name
- Patient date of birth
- Patient date of service
- Clinic address of visit
- Signed Medical Record Release Form
Please provide secure email or fax number for records and allow 5 to 7 days for processing.
All request for medical records must be requested by fax at 410-334-6352. Your Doc’s In clinics will not provide medical records at time of visit nor to patients who walk in after a visit. Patients who are referred to other providers or service centers will have their records forwarded when the referring provider completes the order.
